What services can we fund?

Care Coordination are able to make referrals, based on needs identified or supported by the needs assessment, to a number of support options that are funded by Health NZ | Te Whatu Ora.


Home and community support providers provide supports in the home – particularly around hygiene, dressing, medications etc. These agencies work with a restorative focus


Some people may also qualify for assistance with basic heavy household tasks such as mopping floors and vacuuming. There are a number of additional criteria required for this support, including having a community services card, and not having an able bodied person living in the home, or natural support (someone who could be reasonably be expected to assist) living in the area.

Home Support

Assistance with showering and dressing, and other personal health needs.


Assistance with heavier household tasks – several additional criteria apply.

Carer Supports

Provide opportunities to support/relieve full-time carers including Day programmes, carer support subsidy and community services.

Respite Care

Short term (respite) care in residential facilities to provide relief for carer. Also available in urgent situations when a carer is unable to provide care.

Permanent Residential Care

Assessment criteria apply for all levels of care. Some levels need specialist “sign-off”.  Residential subsidy criteria apply for costs / payments and there may be facility surcharges / premiums.
