How to refer

Anyone can refer to Care Coordination, though for new clients it is helpful if the referral comes from the GP practice, or other health practitioners, as this ensures that all relevant health information is able to be provided.

 

It is not uncommon for follow on referrals to sometimes come directly from family members, or clients themselves.

 

To be eligible for supports, there must be an identified age related heath or disability need.
